Social Worker/Case Manager II

Full Time

SUMMARY

This position is responsible for assessing, planning, implementing, monitoring, and evaluating actions required to meet the consumer’s needs.  

JOB DUTIES

  • Assesses consumer needs based on their needs, limitations, and desires.
  • Develops a case plan with the consumer.
  • Maintains constant communication with the consumer.
  • Maintains consumer’s confidence and protects operations by keeping information confidential.
  • Coordinates and provides services that are client centered.
  • Provides trauma-informed, culturally competent holistic services to victims.
  • Develops and maintains client case records.
  • Handles case assignments, drafts service a plan, reviews case progress and determines case closure.
  • Works with consumers and appropriate community agencies to coordinate and facilitate services for consumers.
  • Assists consumers in making informed decisions by acting as their advocate.
  • Attends court proceedings, if necessary.
  • Actively participates in team review of records and cases.
  • Follows policies and procedures, participates in quality reviews, reporting needed changes.
  • Provides information to the public regarding the program.
  • Provides case management/file documentation for individual caseload and submits biweekly and quarterly reports in a timely manner.
  • Provides outreach and support services, i.e., mobile advocacy.
  • Develops professional relationships with other Federal, State, and Tribal agencies, in order to develop resources for consumers.
  • Performs other duties as assigned.

 NECESSARY KNOWLEDGE, SKILLS, AND ABILITY

  • Knowledge of health, social services, legal issues, and other general/local services throughout the Fairbanks area.
  • Ability to learn and apply program/agency operating policies and procedures.
  • Ability to report to work in a timely manner.
  • Ability to operate computers utilizing a variety of computer software and ability to type. Ability to complete digital and paper forms accurately. 
  • Understanding and sensitivity to diverse cultures and lifestyles, and ability to work effectively with Alaska Native populations.
  • Skill in managing multiple priorities and tasks concurrently and meet deadlines.
  • Skill in oral and written communication. 
  • Skill in establishing and maintaining cooperative and respectful working relationships with children, parents, supervisors, and other employees.
  • Ability to work independently as well as with teams.
  • Ability to effectively present instruction, and/or information to individuals, staff, administrators, and the public.

 MINIMUM QUALIFICATIONS (Education & Experience)

  • Bachelor’s Degree in Social Work
  • Minimum two (2) years of work experience in case management OR
  • Minimum two (2) years of direct service to the targeted population or similar experience
  • Strong understanding and sensitivity to diverse cultures and lifestyles, with demonstrated experience working effectively with Alaska Native population.

 PREFERRED QUALIFICATIONS (Education & Experience)

  • Master’s Degree in Social Work
